What you need for your first visit

Dr. Saatman has a long new patient waiting list. 
Some patients have acute conditions that require
immediate attention while other patients have chronic degenerative disorders.  Dr. Saatman has developed
an office procedure for scheduling patients that
addresses this issue. Your first appointment date
is scheduled based on a review of your medical
history and studies.  In other words, patient’s
appointments are triaged based on their medical
need just as an emergency department treats
severe bodily injuries before common colds.

Before your appointment can be scheduled, you will need to have
the following items delivered to the office.

  • HIPAA Authorization Form (link to forms page)
  • Patient Health History Form (link to forms page)
  • Patient Info Sheet Form (link to forms page)
  • Copy of Insurance Card (front and back) (or bring to office and we will copy for you)
  • Films, Studies, Reports that you have had done.
    Below are examples of these items:
    - X-Rays
    - CT scans
    - myelograms
    - MRI scans
    - EMG/NCV reports
    - bone scans

For your convenience, you can print and complete these forms at home and e-mail them to:

or you can bring them to the office. You can call the imaging center where you had your films
done and instruct them to deliver the films to Dr. Saatman at Bay Area Neurosurgery or you can
bring them to the office.
